To apply for admission into the collaborative criminal justice program, you must complete the online admissions form. While completing this form, you must choose a Home Institution from among the participating schools. This is the school you will earn your degree through; you must follow its graduation requirements. This school also provides you with student services including academic advising and financial aid counseling.
New Student Admission
The admissions form has five sections and may take 10 to 30 minutes to complete. You will provide specific information such as birth city and county, driver's license number, and high school GPA and graduation year. Do not use the back button to correct information; some browsers delete the choices you've made. You will be able to correct information later using an update screen. At the end of the form, follow the instructions to pay your admissions processing fee.
Re-Admitting Student
If you have not taken classes for five semesters or more, you will need to pay a $35 readmission fee. Contact the Service Center via email or by calling 877.533.9235.

Use this form to update information that commonly changes such as email or street address and phone number. Contact the Service Center via email or by calling 877.533.9235 to update any information that does not appear on this form.
Existing Student Changing Major to the
Utah Criminal Justice Collaborative Degree
All changes in major must be done in writing. Contact the Service Center vial email with your request. Make certain to include your full name and home institution in your request. You may also fax a written request to 801.585.3276.
